Job description
Healthcare Support Worker

The post holder will work as a member of the Primary Care Nursing Team under the direction of the lead nurse. The Health Care Support Worker will support Registered Nurses, General Practitioners and other health care professionals in the assessment, treatment and management of patients presenting in primary care settings. The health care support worker will work as part of the nursing team within standard operating procedures, and Registered Nurses and General Practitioners are available for reference. The post holder will assist with specified delegated clinical tasks that include the provision and monitoring of Chronic Disease management providing a caring, patient focused service at all times.

Main duties of the job
  • Carry out delegated clinical duties within HMP & YOI Parc.
  • Undertake Phlebotomy clinics and dry blood spot testing.
  • Perform physiological measurements such as blood pressure monitoring, pulse monitoring, waist circumference measurement, weight monitoring.
  • Providing any necessary Health Promotion advice to patients and carers e.g. Smoking Cessation, Weight Management Support, Alcohol Consumption.
  • Undertake Spirometry testing, and record result in patient information.
  • Undertake ECG monitoring.
  • Participate in the administration of immunisations under a Patient Specific Directive following a period of training i.e. B12, influenza and pneumococcal injections.
  • Perform basic wound management under the supervision of a Registered Nurse following appropriate training and assessment.
  • Prepare the clinical environment for the patient ensuring the treatment area is clean, stocked and tidy.

Welsh Skills Desirable: This post is advertised as Welsh Desirable. This doesn't mean essential; while the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short‑listing and selecting candidates. This isn't 'fluency', just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health. Person Specification
Knowledge and skills
  • NVQ Level 3 in Health and Social Care (or equivalent); Physiological measurements training i.e. Blood Pressure monitoring.
  • IT literate; Proficient in the use of primary care computer software such as "Vision" or "EMIS".
  • Welsh Language Speaking/Listening Skills (Level 3 and above/B2) are Desirable for this Role.
Experience
  • Significant experience of working within Primary Care setting; Previous experience of working within a secure setting.
