Job description
Health Care Support Worker

Band 3 – £24,937-£26,598

30 hours per week, Permanent part‑time (up to 30 hours, negotiable).

We are looking for a motivated and enthusiastic Band 3 Health Care Support Worker to work in the Locala Community Heart Failure Team in Huddersfield.

The service is delivered Monday‑Friday, 8 am‑5 pm. The post holder will work with specialist nurses, supporting holistic care to patients with heart failure, taking blood, observations and other clinical tasks to support patient optimisation, supporting nurses in delivering care, contacting patients, arranging clinics and home visits, organising visits and delegated tasks.

We look for an individual with established phlebotomy and patient observation skills, confident working as part of a team and alone in a variety of settings.
