A healthcare organization seeking a team member to provide support to individuals referred to a community service. The role involves emotional support, assisting with accessing community services, and promoting independent living. To be able to work as a member of a team, to provide support, and give time to individuals referred to PCDU. The team will work with individuals who require short term immediate support to allow them to engage with support networks in their community, the support could be around employment, housing, financial issues, interpersonal relationships or other social issues. The primary focus of the post will be short term work utilising support from ordinary resources within the community and urgent care. To assist Clinicians in the assessment of health and social need of individuals referred to PCDU. Provide emotional support to patients, relatives and carers. Under the direction of clinical staff, the post holder will assist individuals to access community support to enable them to leave the PCDU and use appropriate community services to meet their needs. To assist and engage in practical activities with individuals to enable them to live as independently as possible and promote social inclusion. This will involve working with the individual in short term practical activities such as engaging with housing providers, General practitioners, social care, other secondary care mental health services, voluntary organisations and benefits welfare agencies.
