Health and Wellbeing Practitioner

Location: East Lothian

Salary: £29,616 full-time (pro rata for part-time)

Vacancy Type: Permanent, Full Time

We are looking for a full time Practitioner to join our Wellbeing team located in East Lothian GP Practices.

Qualifications
  • Health and social care practitioner with experience of supporting people living with Long Term physical and mental Health Conditions to self-manage.
  • Capable of supporting vulnerable people who experience significant psychosocial barriers and health inequalities.
  • Belief that people have strengths, are resilient and are experts in their own lives.
  • Proficient in autonomous work in the community setting.
  • Strong solution-focused coaching and group facilitation skills.
Responsibilities

The post holder will:

  • Work with a team of practitioners to deliver our East Lothian GP contract.
  • Travel across East Lothian to engage with people referred to the Wellbeing Service.
  • Practice persistent engagement to help people who struggle to receive support.
  • Manage a caseload of people referred to the service.
  • Use a strengths based personal outcomes approach to help people identify and access appropriate support for them to self-manage, including:
    • Providing specific coaching in self-management.
    • Delivering group based Lifestyle Management courses.
    • Linking people to community resources and supports.
  • Support volunteers who have lived experience of long-term conditions into peer roles.

Location: The practitioner will be based in GP practices across East Lothian and significant travel between GP practices is an essential aspect of this role.
