Job Summary
The Health and Wellbeing Coach is a non‑clinical / medical role and a new service introduced into the PCN through external funding – Sport England Place Partnerships investment. The role supports patients to take control of their well‑being and live happier, healthier lives through a focus on group clinics. You will be required to provide support to patients in a face‑to‑face environment, as well as by email and telephone.

Responsibilities
- Deliver group clinics: plan and facilitate group sessions in community settings to support patients in improving well‑being and sustaining healthier lifestyles.
- Empower patients: promote independence, self‑management, and sustainable behaviour change through personalised support and motivational techniques.
- Facilitate access to resources: connect individuals with peer support, self‑management tools, and community activities to build confidence, resilience, and social connection.
- Collaborative working: partner with Social Prescribers, Care Coordinators, Pharmacists, and wider clinical/community teams, including the Active Partnership, to link patients to local services that promote movement and well‑being.
- Person‑centred support: use coaching and motivational interviewing to help individuals set and achieve meaningful health goals tailored to their needs and preferences.
- Data‑informed practice: use frameworks such as Core20Plus5 to target interventions and support access to personal health budgets where appropriate.
- Monitor and evaluate: conduct regular reviews, maintain accurate records, and contribute to service evaluation using both qualitative and quantitative methods.
- Community engagement: actively participate in local initiatives to promote health awareness and physical activity.
- Professional standards: uphold confidentiality, ethics, and a holistic, person‑centred approach in all interactions.
Qualifications
Essential
- GCSE grade A to C in English and Maths
- Excellent communication, interpersonal, and motivational skills
- Knowledge of health and well‑being principles, including nutrition, physical activity, and mental health
- Ability to work independently and as part of a team
- Strong listening skills, empathy, patience, and a non‑judgmental approach
- Ability to build rapport and trust with individuals from diverse backgrounds
- Understanding of behaviour change principles and motivational interviewing techniques
- Ability to access and utilise relevant resources and information
- Computer literacy and ability to use relevant software and systems
Desirable
- Qualified to NVQ level 2 in Health and Social Care
- Efficiency in writing reports and presenting information in various formats to suit the audience
