Followingthe retirement of one of our long standing GPs, we are seeking a newGP at our innovative GP practice now managed by the GP federation. Wechampion the doctor patient relationship & continuity of care at practicelevel, underpinned by the expertise of the federation & at scale services.

Theworkplan we are seeking to fulfil is for the following 6 sessions:

Monday Duty Day

Tuesday and Wednesday Routine Dayshours can be flexible to allow for approx. 8 hours per day

Thereis also some flexibility for an additional 7th routine session.

Wewould consider this being undertaken by 1 or more GPs.

The Much Birch GMS contract was novated toTaurus Healthare in February 2024, but the practice team remains unchanged asdoes the identity of the surgery inthe local community. Much Birch is :-

  • Abusy rural dispensing practice at the heart of the community.
  • Locatedin the village of Much Birch, between Ross-on-Wye and Hereford city.
  • Coveringan area reaching from the edges of Hereford to Ross on Wye, and extendinginto the villages of the Golden Valley.
  • Listsize of circa 5,000+ patients
  • A supportive clinical team of GPs, ANPs, Paramedics,Practice Nurses, HCAs, Physiotherapist, Social Prescribers
Job responsibilities

Job summary:

The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.

Clinical responsibilities:

  • In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
  • Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
  • Assessing the health care needs of patients with undifferentiated and undiagnosed problems
  • Screening patients for disease risk factors and early signs of illness
  • Developing care plans for health in consultation with patients and in line with current practice disease management protocols
  • Providing counselling and health education
  • Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
  • Recording clear and contemporaneous consultation notes to agreed standards
  • Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)
  • Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
  • In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.

Qualifications
  • Basic medical degree and completion of GP vocational training or equivalent (JCPTGP) certificate.
  • MRCGP.
