Qualifications

• Minimum of 1 year experience in home health, rehab, or clinical setting preferred.
• CPR certification and up-to-date health requirements.
• Reliable transportation and willingness to travel to patient homes.

Responsibilities

• Conduct in-home assessments of patients' abilities.
• Develop individualized treatment plans for patients.
• Provide direct therapeutic interventions and training.
• Educate patients and caregivers on safety strategies.
• Recommend home modifications as needed.
