Qualifications

• Experience in carrying out initial risk and needs assessments in service users’ homes or other settings such as hospitals or residential/nursing homes.
• Ability to work in partnership with service users, family carers, advocates, healthcare professionals and others to ensure seamless delivery of holistic care packages.

Responsibilities

• Support the delivery of quality care support services through assessment and supervision of care workers.
• Plan effective care worker rosters in collaboration with Care Coordinators.
• Conduct regular reviews and risk assessments of individual services.
