Job summary

Are you a qualified Mental Health or Learning Disability Nurse who is interested in making a difference, is passionate about working with people with a learning disability and/or autistic adults, and enjoys working in a fast paced environment with creative dynamic approaches to your work? If so, this is the job and team for you.

The Enhanced Support and Liaison Service (ESLS) is a newly formed service that supports adults with learning disabilities and/or autistic adults who are at risk of placement breakdown, at risk of hospital admission, and/or who need support to access mainstream mental health services.

The LDA Enhanced Support & Liaison Service is a county-wide, 7 day per week service which functions as part of a multi-agency pathway, working with service users, providers, carers, families, social care, and health partners to enable people with a learning disability and/or autism in Lincolnshire to receive appropriate care in the right place at the right time. The LDA Enhanced Support & Liaison Service offers the following levels of support:

  • Level 1 - Generic support
  • Level 2 - Patient Specific Support
  • Level 3 - Enhanced Patient Specific Support

This support is offered through the following pathways:

  • Community Liaison Pathways
  • Inpatient/discharge Pathway
  • Enhanced and intensive community Intervention & Support

Main duties of the job

You will be part of the enhanced intensive pathway within ESLS, responsible for urgent assessment, including risk assessment, planning, and implementation of evidence-based care to service users with Learning Disabilities and/or Autism. Colleagues within the enhanced intensive pathway work as part of a 7-day-a-week roster, doing 08:00 - 20:00 shifts.

ESLS works with teams and services across the trust to provide support and intervention for those who are at risk of admission or who are at risk of placement breakdown. This support is primarily through liaison, advice, training, and consultation with other trust services and with wider system partners including the Integrated Care Board (ICB), Lincolnshire County Council, United Lincolnshire Hospitals Trust, Police and Voluntary and Third Sector Organisations.
