EHC Case Coordinator (0-25 SEND)

We are seeking a dedicated and detail-focused Education, Health and Care plan case Coordinator (EHC) to join our team supporting children and young people with special educational needs and disabilities (SEND) across the 0-25 age range.

This role is ideal for individuals with strong case management skills, excellent communication abilities, and a thorough understanding of statutory SEND processes.

Key Responsibilities
  • Coordinate statutory Education, Health and Care (EHC) Needs Assessments for children and young people aged 0-25.
  • Manage a defined caseload of children and young people with EHC Plans, ensuring their needs are met within their educational provision.
  • Lead and complete the statutory annual review process for EHC Plans.
  • Prepare accurate and professional written briefs and provide oral representation to EHC Panels and other relevant forums.
  • Maintain and update all case records in line with GDPR, the Data Protection Act 2018, and London Borough of Barking & Dagenham data policies.
  • Attend regular supervision sessions to ensure case progression aligns with decisions made at Local Authority Panels.
