Overview

We are currently receiving significant investment aiming to transform community mental health services across the Black Country. Are you ready to take on an exciting role at the forefront of adult community mental health services in Sandwell?

If you are someone who is innovative, proactive and passionate about providing high-quality holistic care, then do not miss your opportunity to apply for this exciting new role of Dual Diagnosis Nurse within our Community Mental Health Team (CMHT).

We are developing an integrated community model, with the focus on new ways of working within integrated mental health teams to support people living in their communities with mental health challenges.

Role and Responsibilities

You will be a Registered Mental Health Nurse, with experience of working in community settings with Service Users with complex mental health and substance misuse issues. You will be responsible for a caseload of clients providing clinical care and guidance for those with drug and alcohol dependency and to support outreach to substance misuse services.

Experience in assessing and formulating individual intervention plans and outcome measures for adults with complex mental health problems and have current knowledge of NHS policies and practices in mental health. You will participate and support the CMHT duty system and be responsible for managing the mental health needs of Services Users referred to the team.

Qualifications and Experience

Successful applicants will have previous knowledge of the recovery agenda and experience of working with a focus on resilience and confidence based prevention to promote better outcomes for individuals. Have a working knowledge of psychotropic medication and effectiveness plus the ability to deliver a range of therapeutic interventions. In addition, the post holder will demonstrate the ability to undertake and manage clinical risks and develop robust clinical management plans for Service Users with the service.

Team and Service Delivery

You will support the Team Manager and Clinical Lead to adhere to and deliver high quality service and have in depth knowledge of national quality indicators.
