Job Purpose

To provide high-quality, person‑centred care and support to individuals in their own homes, enabling them to maintain independence, dignity, and quality of life.

Key Responsibilities
Personal Care
  • Assist clients with personal hygiene, including washing, bathing, dressing, and toileting
  • Support with mobility, transfers, and use of mobility aids
  • Assist with continence care and skin care
Medication Support
  • Prompt, assist, or administer medication in line with care plans and company policies
  • Maintain accurate medication records
Daily Living Support
  • Prepare meals and assist with feeding where required
  • Support with light household tasks such as cleaning, laundry, and shopping
  • Assist with appointments and social activities
Emotional & Social Support
  • Provide companionship and emotional support
  • Encourage independence and social interaction
  • Promote mental wellbeing and dignity at all times
Care Planning & Record Keeping
  • Follow individual care plans and risk assessments
  • Maintain accurate and timely care records
  • Report any changes in a client’s condition to the appropriate person
Health & Safety
  • Follow safeguarding policies and procedures
  • Adhere to infection prevention and control measures
  • Report accidents, incidents, or concerns immediately
Communication & Teamwork
  • Communicate effectively with clients, families, and healthcare professionals
  • Work collaboratively with colleagues and management
  • Attend training, supervision, and team meetings as required
