We are seeking a Community Nursing Team Leads to work within the Mid Bedfordshire Locality.

You will be a strong leader, focused on achieving real improvements in outcomes for patients and will be confident in your ability to support, coach and develop individuals and teams to deliver consistently high quality services. You will be innovative, forward thinking, self-driven with excellent communication and interpersonal skills.

Main duties of the job

You will take pride in your work and be highly motivated and dynamic, have a flexible approach and be able to embrace change. Being a collaborative worker and the ability to network and build relationships with a diverse group of stakeholders is essential. You must hold the DNSPQ qualification, or be working towards this.

You will provide day to day operational and line management of community nursing teams, work alongside clinicians to support complex patient reviews and mentor nurse colleagues.

Job responsibilities

The successful applicant will be managing two caseloads and the aligned staff, working alongside the existing team lead and service manager to support and manage the delivery of high quality person centered care. One post will be based in Shefford Health Centre, aligned to the Shefford Community Nursing teams and the second in Steppingley Hub aligned to the Ampthill Community nursing teams.

Due to the nature of the post the applicant must be able to meet the travel requirements of the post.

Education and training
  • Registered General Nurse
  • Educated to degree level or equivalent
  • Post-registration qualification in clinical field eg: SPQDN (or working towards SPQDN)
  • Nurse Prescriber
  • Relevant teaching/mentoring qualification and evidence of teaching skills
  • Demonstrable evidence of significant post-registration nursing experience in a community setting.
Experience
  • Evidence of and commitment to multi-professional working
  • Demonstrate working within a multi-agency environment.
  • Have a comprehensive understanding of partnership working.
  • Demonstrate application of clinical governance
  • Excellent understanding of community health and social care principles and practice
