Job description
Overview

An exciting opportunity has arisen for a District Nurse or Community Nursing Sister to support in the leadership of the Bridport and Lyme District Nursing Service. The team works with other health and social care professionals across primary and secondary care, as well as with patients, families and carers in their own home.

Responsibilities
  • Lead the team and caseload to provide care within individuals' homes or residential settings, including palliative care and long-term conditions management.
  • Deliver holistic care to adults with physical health needs, using autonomous initiative.
  • Provide supervision, mentorship, training and clinical support to students and trained practitioners.
  • Develop specialist programmes of care for other professionals to deliver and ensure they are delivered appropriately.
  • Plan, implement and review health improvement programmes in various settings.
  • Provide specialist care, interventions, assessments and treatments, and manage patients on a caseload as part of a wider multi-disciplinary care team.
  • Give specialist advice on the management and care of patients/service users.

Employment Details

The role is full time and will include working one weekend a month; part-time hours will be considered.
