Job summary

A leading healthcare organization in Southampton is seeking a Digital Health Care Assistant to support patients on a digital care pathway. This role involves acting as a liaison for patients, coordinating their care, and ensuring efficient service delivery. Candidates should have an NVQ Level 3 in Health and relevant caring experience. The position offers a salary range of £27,485 to £30,162 annually, pro-rata, within a supportive and inclusive environment.

Qualifications

  • NVQ or apprenticeship Level 3 in Health or equivalent experience is required.
  • Numeracy and literacy skills at level 2.
  • IT literacy and proficiency in hospital systems.

Responsibilities

  • Act as a single point of access to patients using My Medical Record.
  • Support and coordinate patients on virtual pathways.
  • Proactively manage patient needs and escalate issues as necessary.

Main duties of the job

An exciting opportunity to join our IBD team at UHS. The service provides complex, long‑term care to patients with chronic conditions requiring ongoing management, coordination and multidisciplinary input.

This post is integral to ensuring that this care is delivered safely, efficiently and compassionately.

Job responsibilities

Act as a single point of access to patients who are being registered to use My Medical Record as part of their care.

Proactively support and coordinate for the overall needs of patients who are on a digital virtual care pathway using own judgement and escalating to the relevant practitioner as necessary.

Person Specification
Qualifications / training required
  • NVQ or apprenticeship Level 3 in Health or equivalent experience
  • Numeracy skills and Literacy skills at level 2 (or equivalent).
  • IT literate
  • Proficient in use of Hospital systems
  • Experienced and competent in additional clinical skills, as per job description.
Previous or relevant experience necessary
  • Proven experience in a caring role
  • Understanding of acute hospital environment and the role of SHCA
  • Previous experience of working with the general public
  • Experience of using a computer
  • Previous HCA experience
  • Experience working with diverse audiences (eg. volunteers, patients and/or community groups).
Aptitudes and skills required
  • Ability to communicate with wide range of people
  • Ability to work as a team member
  • Ability to work under own initiative and prioritise workload
  • Ability to adjust to pressures of jobGood interpersonal skills
