Overview
We are recruiting a Band 7 Diabetes Clinical Specialist Nurse to work within the Sefton Place Community Diabetes Team. The post holder will need to demonstrate a comprehensive knowledge base in relation to diabetes and promote excellence in nursing practice in a dynamic healthcare environment. The post holder is expected to demonstrate leadership values within the team and contribute to the delivery of specialist nursing care, case management, and provide direct patient care, acting as an expert practitioner to optimise patient health, provide education for self-care, and support the complex management of patient pathways.
Eligibility and progression
We welcome applications from candidates who have the required experience in diabetes management but do not currently hold the appropriate prescriber (V300) qualification. In such cases, applicants would need to be willing to commence the course within 6 months of starting in the post which would be supported by the Trust. These applicants would commence on Band 6 with a clear progression plan to Band 7 upon achievement of the required qualification, competencies and experience, in line with Mersey Care’s commitment to supporting staff development.
Responsibilities
- Provide highly specialised clinical advice and support to service users and the wider healthcare team – this will include clinical assessment, diagnostic skills, non-medical prescribing and implementation of management and treatment plans.
- Be responsible for the delivery of the service within the area and work cooperatively and collaboratively with other services/stakeholders to enable service users are treated in the most appropriate setting.
- Ensure that the team provides a high quality service to its clients by providing caseload and clinical supervision and ensures proactive delivery of anticipatory and maintenance care for planned and unplanned needs.
- Maintain clinical credibility by providing clinical care and supporting members of the team within the clinical environment; participate in the development of caseload management across the local health economy.
- Provide leadership and mentoring to staff developing into a caseload management role and act as an advocate for patients/clients in various forums and professional groups.
- Implement plans for the team including rotas and schedules/working patterns to ensure business continuity; practice autonomously with evidence-based clinical decision making.
- Provide clinical expertise and knowledge to the wider healthcare community when managing complex and highly complex situations; assess patient/client conditions and consider a range of options in delivering care.
- Develop specialist individualised care plans following holistic assessment with involvement of patients/clients and carers; implement and evaluate care delivery for identified needs.
- Ensure that clinical activity reflects health promotion, supported self-care, disease-specific management, long-term conditions management, and end of life/palliative care objectives.
- Set objectives for performance monitoring and collaborate with the team leader and service lead to deliver local services; provide reports on staff and patient activity as requested.
- Work with others to support practice development and service modernisation; contribute to role and service redesign and policy development as required.
- Provide induction to the local working environment, and policies for new team members and students; act as an authorised signatory and ensure probity in timecards and mileage claims.
- Monitor budgets, support team leader with PACE reviews, and contribute to policy and service development to reflect local needs.
- Delegate administration and clerical duties appropriately and provide data to monitor contract and objectives; participate in audits, research, supervision, and mentorship initiatives.
- Initiate training and development of team members; monitor standards of patient care delivery, clinical competency and records; ensure ongoing review of care provision with the team leader and service lead.
- Work within guidelines to identify and manage risk, report incidents per Trust policies, and undertake investigations as delegated; ensure quality of patient care through audit, caseload and clinical supervision; participate in patient satisfaction reporting.
- Maintain professional registration; support mentoring of students; identify skills deficits and methods to address them for service improvement and development.
