Overview

We are recruiting a Band 7 Diabetes Clinical Specialist Nurse to work within Sefton Place Community Diabetes Team. The post holder will demonstrate a comprehensive knowledge base in relation to Diabetes and promote excellence in nursing practice in a dynamic healthcare environment. The post holder will demonstrate leadership values within the team and contribute to the delivery of specialist nursing care, case management and direct patient care, acting as an expert practitioner to optimise patient health, provide self-care education and manage complex patient pathways. The role is based in Sefton Place, delivering a service to patients in Southport, Formby and the South Sefton area. The post holder may undertake leadership duties in the absence of the Team Leader and should be able to lead a team effectively and work autonomously.

Responsibilities
  • Deliver specialist nursing care and case management as part of a multi-disciplinary team, acting as an expert practitioner to optimise patient health and provide education for self-care and complex management of patient pathways.
  • Provide highly specialised clinical advice and support to service users and the wider healthcare team, including clinical assessment, diagnostic skills, non-medical prescribing and implementation of management and treatment plans.
  • Lead and mentor team members in caseload management and supervise clinical activity to ensure high-quality service delivery.
  • Lead in the absence of the Team Leader, contribute to service development, policy development, and integration with other services.
  • Develop specialist, individualised care plans following holistic assessment in collaboration with patients and carers; implement and evaluate care delivery for identified needs.
  • Ensure the service delivers health promotion, supported self-care, disease-specific management, long-term condition management and end-of-life/palliative care objectives.
  • Set performance objectives and monitor delivery; work with the Team Leader and Service Lead to deliver locally based services and report on outcomes.
  • Collaborate with other services/stakeholders to enable care in the most appropriate setting and maintain a high-quality service through caseload management and clinical supervision.
  • Advocate for patients/clients and participate in policy and service development, including initiating training and development for team members.
  • Provide induction and ensure team members and students are oriented to the local working environment and policies; act as an authorised signatory for timecards and mileage claims where applicable.
  • Support budgets, workforce planning, mentorship, and clinical supervision, and monitor administrative duties delegated to clerical staff.
  • Maintain clinical credibility by delivering care within evidence-based practice and contributing to quality improvement through audits and patient satisfaction reporting.
Qualifications and Requirements
  • Demonstrated leadership/management experience and ability to work autonomously within a busy diabetes service.
  • Comprehensive knowledge of diabetes care and the ability to provide expert nursing practice and complex management of patient pathways.
  • Non-medical prescribing (where applicable) and ability to deliver high-quality clinical care in a community diabetes setting.
  • Experience working within a multi-disciplinary team and ability to collaborate with external services/stakeholders.
  • Strong communication skills and the ability to provide education to patients and carers; capable of supporting practice development and service modernisation.
