We are a dynamic and forward thinking team looking for a band 6 nurse to join our established St . Albans & Harpenden Nursing team as a Deputy Team Lead

A great opportunity has arisen for an experienced and motivated registered nurse to lead one of our neighbourhood teams under the direction and leadership of the Team leaders

Your role would be varied and interesting carrying the responsibility for a patient caseload which involves the assessment, planning, implementation and evaluation of evidence based care to patients in their home environment. Previous community experience is essential.

The successful applicant must be someone with a strong commitment to working as part of a team, be approachable, a good communicator, have the ability to work autonomously and be receptive to change.

Main duties of the job

This is an exciting time to join the community nursing service in West Herts. We are developing new community pathways which will support the delivery of more care in people's homes, and to enable us to do this we have a range of new learning and development opportunities available through the CLCH Academy.

  • To set and maintain standards of care and deploy, co-ordinate and supervise the nursing team and learners as required.
  • To provide leadership and management to a team of community nurses and other staff covering the four localities across Herts Valleys.
  • To be responsible for the team's assessment, planning, implementation and evaluation of individualised nursing care to patients living in the community. The service is provided seven days per week 8am-8pm
  • To be responsible for the team's maintenance of patient and other records
  • To participate in the long term management of patients within the locality
  • To have continued responsibility for a defined caseload

Education/Qualification
  • Registered Nurse with current UK NMC registration
  • Practice Supervisor/Assessor or equivalent
  • Specialist Nursing Practice (District Nursing) Degree or will undertake training
  • Nurse Prescribing or will undertake training
  • Evidence of personal development
  • Knowledge of NMC code of professional conduct
  • Independent Prescriber is Experience
  • Relevant courses in Long Term Conditions Management
  • Course of Top to Toe examination/ diagnosis Experience

  • Experience Community Nurse

  • Proven clinical skill at Senior Community Nursing level
