Qualifications

  • Must pass an Enhanced Disclosure and Barring Service (DBS) check.

Responsibilities

  • Provide specialist and intensive short-term support at home for dementia patients.
  • Support discharges of people with dementia from wards into community placements.
  • Collaborate with other health and social care agencies.
