Job Title: Dementia Care Manager - Hutton Care home

Hours: 40 hours per week

About the role

As a Dementia Care Manager, your prowess and magical touch will lead and manage a committed and loyal caring team on the community and be responsible for driving all aspects of the Hallmark dementia strategy, ensuring your community meets the stringent standards we have specially created for excellent relationship-centred dementia care.

With a focus on developing people, rather than ‘task’, it goes without saying you must have a passionate soul for dementia care, with the confidence to challenge the status quo that create ill-being for the people in your care.

Supported by our Head of Relationship Centred Care and Dementia Relationship Practitioner, you will be the smiling face “go-to person” for our residential and nursing communities with expert knowledge of delivering exceptional evidence-based care in the home. A committed approach will forge and build links with local community groups involved in dementia care – becoming the Hallmark ambassador for dementia in your local area.

Being a friendly jolly character and empathetic, will allow you to support family members and friends of people living with dementia to become partners in the care of their loved one.

Alongside, responsibility will be to ensure the dementia community meets all the requirements of the CQC, Hallmark’s quality and compliance framework, right through from assessment, care planning, quality assurance, health and safety, and recruitment and management of your team.

Solid relationship bonds will develop reporting to and working in collaboration with the General Manager of the home and other internal Heads of Department.
