Critical Care Outreach Nurse

The Band 7 senior sister for critical care outreach will be a senior member of the critical care outreach team. They will possess expert clinical skills in the assessment and treatment of acutely unwell and deteriorating patients. They will deliver a high standard of care to patients in ward areas, seeking to avert admissions to Intensive Care or facilitate prompt and safe transfer. They will assist with safe patient discharge from Intensive Care, providing input into rehabilitation and providing psychological support for patients and relatives. They will provide professional and educational support to colleagues working in other clinical areas. Active participation in audit, quality improvement and research is a key requisite of the role.

Main duties of the job
  1. Act as a self‑directed, autonomous clinical practitioner who can effectively prioritise own workload and that of others in the clinical arena.
  2. Be a confident, reflective clinician demonstrating skill in critical decision making and problem solving.
  3. Possess expert specialist knowledge and demonstrate excellence in clinical competence in a variety of hospital scenarios and settings.
  4. Respond to track and trigger scoring, enabling timely recognition of deterioration in acutely ill patients.
  5. Respond rapidly in an appropriate manner to all medical emergency and cardio‑pulmonary arrest calls within the hospital setting.
  6. Effectively perform advanced patient assessment based on history taking and clinical presentation.
  7. In liaison with the multi‑disciplinary team, initiate appropriate diagnostic intervention and accurately interpret and act upon clinical findings.
  8. In liaison with the multi‑disciplinary team, set appropriate goals for patient management and skilfully participate in the implementation and evaluation of all therapeutic interventions.
  9. In collaboration with the multi‑disciplinary team, participate in patient stabilisation prior to and during the transfer process to another clinical area.
  10. Demonstrate excellent communication skills with all professional colleagues, making appropriate and timely referrals where necessary and working collaboratively to set goals.
  11. Demonstrate excellent communication skills with patients and their families, providing sensitive, appropriate support and consistent and realistic information.
  12. Maintain accurate records, effectively documenting patient assessment, care planning, implementation and evaluation.
  13. Always practice in accordance with the NMC Code of Professional Conduct and Trust policies and protocols.
Education
  • Maintain current professional registration with the NMC.
  • Maintain current mentor status with the NMC.
  • Ensure that own clinical practice is evidence based and that all requisite academic knowledge is obtained and regularly updated.

  • Contribute to the ongoing provision and development of critical care outreach services within the Trust.
  • Creates and maintains effective communication channels both within the outreach team and for the team's interaction with other multi‑disciplinary colleagues.
  • Provides a safe working environment for outreach team members and ensures sufficient resources are available for service delivery.
  • Facilitates teamwork and ensures that the delegation of responsibility to team members is appropriate and managed effectively.
  • Effectively manages and evaluates change in clinical practice.
  • Works collaboratively with senior multi‑disciplinary colleagues within the Trust and attends all relevant meetings impacting acute care provision.
  • Acts as a clinical role model and a learning resource for nursing colleagues in ward areas.
  • Acts as a knowledgeable, highly skilled leader and learning resource for more junior members of the critical care outreach team.
  • Participates in the recruitment of critical care outreach team members and provides all new staff with effective induction to the role and workplace.
  • Effectively appraises junior team members and facilitates their professional development.
  • Effectively monitors junior team members’ clinical performance and implements supervision and additional training where necessary.
Research, Development and Improvement
  • Ensures the practice of all critical care outreach team members is evidence based, ensuring current research and best practice guidelines are always implemented.
  • Leads clinical auditing of the critical care outreach service and actively participates in research activities.
  • Identifies areas for improvement, both within the critical care outreach team and in the clinical area, and uses recognised quality improvement methods and practices to pursue continuous improvement.
  • Identifies potential areas for nurse‑led research within the outreach service and actively participates in research activities.
  • Supports junior team members in auditing and research activities.
  • Leads the presentation of audit and research findings to multi‑disciplinary colleagues within the Trust and externally.
  • Contributes to the publication of audit and research material nationally and internationally.
  • Participates and contributes to regional critical care network activities.
  • Contributes to Trust strategies for nursing and clinical governance.

Person Specification – Critical Care
  • Post registration qualification in Critical care Nursing or equivalent Acute Care.
  • Physical Assessment course.
  • Minimum of 2 years experience at Band 6.
  • Experience of providing formal teaching.
  • ALS Provider.
  • Non‑medical Prescriber.
  • Previous CCOT experience.
