Job description
Case Worker – Connecting Communities

Change Grow Live provide psychosocial interventions and transitional services across prisons in Kent. This contract includes the Connecting Communities team who prepare people engaging with CGL and Oxleas for release back into the community or when transferring to another site. Dedicated staff will work together with healthcare colleagues supporting clients when transitioning to communities across the UK. Our enhanced offer will include flexibility to engage with clients for a period of time post release.

We are looking to recruit a passionate and committed individual to the role of Case Worker within the Connecting Communities Team based within the following three prisons.

This role is based across 3 prison sites, HMP Elmley, HMP Swaleside & HMP Standford Hill.

Key qualifications
  • Excellent understanding of drug and alcohol issues and experience of working within a related field.
  • Commitment to multi‑agency working and establishing partnerships with other professions to achieve good outcomes for the service user group, e.g. mental health services, criminal justice agencies, community groups, etc.
  • Accomplished written and verbal communication skills and a high degree of personal IT competency. Ability to accurately update and maintain records in a timely fashion and to work to deadlines for the submission of information, e.g. reports.
  • Excellent team working and interpersonal skills, maintaining a highly cooperative approach to supporting colleagues in delivering service objectives.
  • Experience of working within a prison/criminal justice setting.
  • Sensitivity in working with complex and diverse cultural issues.
Salary & Hours

Full Time Salary Range: £27,861.26 to £32,002.35

Full Time Hours: 37.5 per week. Contract: Permanent.
