Are you passionate about supporting patients in maintaining independence and maximising health outcomes? Do you thrive in fast-paced environments and have a strong commitment to patient‑centred care? We are looking for a dedicated and dynamic Community Support Practitioner Worker to join our urgent community response and intermediate care team working in Scarborough and Ryedale community services. You will work alongside physiotherapists, occupational therapists and nurses to support carrying out rehabilitation programme's and functional activities to support patients with their rehabilitation in the community. This post involves working with patients with a wide variety of pathologies and medical conditions, who require a period of support to help improvetheir mobility and independence. You will receive regular supervision and opportunities for development along with a robust induction package.

Day to day, the role includes the following care carried out in the patient's own home with supervision/delegation of therapists/registered nurses:
  • Patient care following a fall
  • Moving & handling of patients
  • Vital observations
  • End of life care and urgent support for people living with frailty
  • Recognise and escal… issues of concern and/or safety
  • Assisting with activities of daily living, e.g., personal care tasks, meal preparation following injury/illness to prevent hospital admission- supporting and monitoring mobility, and rehabilitation programmes which include aspects of nursing, occupational Therapy and Physiotherapy.
  • Urgent equipment provision, supporting and guiding the patient and carers with the safe use of this
