Community Staff Nurse Evening & Overnight Band 5

Our service works across the 5 neighbourhoods of Tameside. You will have the support of our induction facilitator alongside a 'buddy'. This is a 24 hour service & the role may require you to work across the localities as a service need requirement.

If you are a self‑motivated, individual with a passion for community care, this could be the position for you. The department are unable to support newly qualified staff at this time.

Main duties of the job

The post holder will work within a Multi‑Disciplinary Team in the Community and after period of induction will be responsible for the assessment of patient care needs and development, implementation and evaluation of programmes of care, both for a specific group of clients as the named care provider and in supporting the care of all client groups within the community. You will act in accordance with locally agreed standards of care and ensure that clinical procedures are correctly carried out. As an experienced nurse and as appropriate, you will co‑ordinate the case load in the absence of the team/senior lead ensuring that effective communication is maintained at all times, providing support for junior staff within the team. You will pay a key role in the supervision and assessment of pre and post basic learners and unregistered staff providing a teaching role and acting as named supervisor.
