Community Staff Nurse

We are offering an exciting opportunity to join us as a NMC Registered Community Staff Nurse to work in our busy and supportive Rugeley & Great Haywood Community Nursing Team based at Sandy Lane Health Centre in Rugeley.

Many people would be unable to live home without the support they receive from the community nursing teams. The community services are admitting patients with a more complex acu than ever before, with increasingly specialist treatment and care being provided by highly skilled staff.

Our community nursing teams assess, plan and deliver nursing care to patients within their own homes, in line with evidence-based practice. It is a privilege to care for patients in their own homes, and by joining our team you will be able to experience the difference this makes to our families within the community.

Due to the nature of community working, you will be required to travel efficiently and independently across a large geographical area to meet the needs of the role.

Main duties of the job

As part of your role as a Community Nurse you will demonstrate commitment to providing high standards of holistic care to our patients.

You will be competent, or willing to gain competency in skills such as palliative & end of life care, long term condition management, diabetes care, medication administration, complex wound care and bladder & bowel management.

Working in collaboration with the multi-disciplinary team and colleagues you will be expected to:

  • Promote independence and support patient centred care planning.
  • Provide care directly to patients on the caseload with a range of nursing needs (including palliative care, complex wound care, pelvic health, medication administration).
  • Work autonomously, in line with your scope of practice, competence and policies.
  • Handover patient information to the wider team, document patient information effectively.
  • Escalation of patients' needs as appropriate.
  • You will receive support into the role through induction, training and beyond. There will be opportunities for personal development and career progression if desired.
