Job description
Overview

Community Staff Nurse – closing date 12 October 2025.

This role is within the Urgent Care Services West Kent, which provides a two-hour response to patients in crisis at home. Our aim is to prevent unnecessary hospital admission and facilitate early discharge from acute hospitals. We also provide unscheduled care out-of-hours for patients who may be at the end of life or experiencing an urgent need for healthcare. This may include treating acutely unwell people that would otherwise require hospital admission, and also supports hospital discharges. The service operates 24 hours a day, 365 days a year. Nurses use a wide clinical skills base to provide hospital-level care at home, including IV therapy, end-of-life symptom control and management via syringe driver, catheterisation skills, phlebotomy, wound management, risk assessment, and general observations and fundamental nursing care. This service provides nursing care within patients\' homes and covers West Kent, so driving is essential to this role.
