Overview

Responsibilities

You will be responsible for providing skilled nursing care and will work flexibly supporting the team to deliver high-quality integrated care.


You will need to demonstrate excellent communication and interpersonal skills with the ability to be adaptable, reliable, resourceful and approachable.


Providing integrated care following a holistic assessment which enables patients to be cared for in their own homes and clinic settings. You will facilitate the prevention of unnecessary hospital admission/re‑admission by reducing the effects of ill health and disability and maximising the individual’s potential for independence.


Qualifications

We are looking for an experienced registered adult nurse to thrive in our community Nursing workforce. You will become an expert clinician in wound care, palliative care, continence care and the care of patients with long term conditions.

Our service operates from 8 am to 8 pm, seven days a week, offering flexibility in working hours. An additional team of Nurses and HCA’s offer a robust, reactive evening and night Nursing team.
