Job description

We have a band 3 Community Support Worker position in our Community Response Team based at Walton Hospital, Chesterfield.

Our team provides vital care and support to patients in their own homes, particularly following a crisis or after discharge from hospital. This service ensures that individuals receive the appropriate care and assistance they need to recover and maintain their independence in a familiar environment.

We work collaboratively across a large geographical area and maintain close links with our other planned and rapid teams in North East Derbyshire and Chesterfield to deliver seamless, patient‑centred care.

Main duties of the job
  • To provide holistic care to patients in their own homes, under the supervision of the appropriate registered professional.
  • To communicate information effectively.
  • To report and take appropriate action regarding any change in patient condition or circumstance and communicate verbal and written information to all relevant integrated care team members.
  • To maintain and record accurate health records electronically, in line with DCHS policies.
  • To participate as a team member in a wider integrated team, in continuous learning to develop your own knowledge and skills.
  • To have a level of understanding with regards to the importance of safeguarding.
  • Respect the dignity of individuals.
  • Obtain consent when involving the patient, their carers/families/relatives in shared decision making.
  • To undertake all aspects of clinical duties and procedures which are documented using SystmOne.
