Are you a qualified nurse passionate about nursing? We are looking for an enthusiastic and compassionate Registered Nurse to join our community nursing team, covering Ashburton, Buckfastleigh, Dartmouth, and Totnes. Strong communication is essential, as we work closely with a wider multi‑disciplinary team, particularly our Intermediate Care Team, to support seamless, integrated care.

Main duties of the job
  • To deliver high quality care to the housebound patients of Totnes, Ashburton, Buckfastleigh and Dartmouth.
  • Work as part of a multi‑disciplinary team and in partnership with primary care and secondary care.
  • Provide and receive complex, sensitive, and contentious information using empathy and reassurance skills.
  • Use motivational and negotiation skills where consent or co‑operation is required, and undertake best‑interest decisions.
  • Ensure timely and effective communication with the team manager and other team members on professional matters.
  • Liaise with patients or relatives who may be unpredictable or challenging in their responses because of stress‑related problems.

  • Experience in complex wound management / compression
  • Experience of continuing health care assessments
  • Previous community experience
Knowledge and experience
  • Proven relevant experience in a range of clinical skills including complex and specialist skills
  • Relevant I.T. knowledge/experience
  • Awareness of mental capacity act
  • Awareness of resource management
  • Holistic client assessment using an identified model of care
  • Utilising specialist knowledge across a wide range of work procedures and practices underpinned by theoretical knowledge
  • Ability to make sound clinical judgements and decisions in response to complex or crisis situations
  • Maintain timely and accurate written records
  • Proven relevant experience in communicating complex information to patients, carers, and health and social care staff
  • Awareness of safeguarding adults and children
