Community Psychiatric Nurse for Younger People with Dementia

An exciting opportunity has arisen for a Band 6 Community Psychiatric Nurse to work within the Conwy Community Mental Health Team for Older Persons.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Main duties of the job

The post is to work with younger people under 65 years with dementia. The CPN will be responsible for assessing, implementing and evaluating care and the risk management of persons who live in the Conwy and Denbighshire area. They will work closely with the Consultant Psychiatrist, Social Services, and in-patient wards as well as third sector and voluntary agencies to ensure the patients are receiving the right care in the right environment.
