Gedling & Hucknall Community Mental Health Services provides assessment and treatment for older people with mental health difficulties and for people of any age with organic diagnosis within the Gedling & Hucknall locality. The County Dementia Outreach Service operates within a “virtual team” model with clinicians based in local community mental health teams. We use dementia-friendly, non-pharmacological, person-centred care approaches, working with residents, their loved ones and care home staff to enhance wellbeing, reduce distress and behaviours of concern, and maintain appropriate care placements. An informal chat or insight visit is welcome.

Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. The salary is below the minimum salary required for sponsorship for a Skilled Worker / Health & Care visa. The Trust would not be able to sponsor for a Skilled Worker / Health & Care visa in these circumstances.
An experienced, NMC Registered Band 6 Community Psychiatric Nurse with substantial experience of working with people with dementia in inpatient/residential or community settings is sought.
Advising and supporting care home staff to improve skills and confidence to manage behaviours and promote person-centred care planning is a key aspect of the role. Specialist knowledge of nursing issues related to dementia in a residential setting, and strong written, verbal and IT skills, are essential. A robust understanding of risk assessment and management and experience of managing a caseload are required. Demonstrate understanding of the mental and physical health and social needs of the client group, safeguarding issues and Deprivation of Liberty Safeguards (DoLS). Time management and organisational skills and the ability to work under pressure are important. The role includes nursing student mentorship.

We are looking for a committed Band 6 Community Psychiatric Nurse to support Gedling & Hucknall within the locality community mental health team for older people. You will be based within the Multi-Disciplinary Community Mental Health Team while also being part of the wider outreach service managed by a clinical specialist.
A key aspect of the role is advising and supporting care home staff to improve skills and confidence to manage behaviours and promote person-centred care planning. The County Dementia Outreach service covers the County of Nottinghamshire and operates within a “virtual team” model with clinicians based in locality teams. Applicants must be NMC registered.
Requirements include a full UK driving licence and vehicle for business use (reasonable adjustments available under the Equality Act 2010). DBS check costs (£26.40 standard, £54.40 enhanced) will be deducted from salary over the first two months. Enrolment in the DBS Update Service is encouraged (£16 per year).
