Main duties of the job

What youll be responsible for:

Providing robust assessment of patients (by telephone or face-to-face) who have beenreferre to the CMHT due to presenting with mental health difficulties.

Identifying appropriate care pathways and ensuring the least restrictive option to meet theneeds of the patient and ensuring their wellbeing.

Signposting to appropriate care providers (including primary and tertiary care). Attending dailyRed Board meetings.

Raising concerns to senior staff for support and management. CompletingRiodocumentation and planning care for patients.

Presenting cases to Multi-Disciplinary Team for discussion around care pathway.

Liaison with colleagues nursing, occupational therapy, medics, psychology and admin
