Job description

We are looking for Community Nurses to work in and around the St Ives/Hunts locality, you will be fully supported to gain new skills and experience needed to deliver high quality nursing care to patients in their own home or residential setting.

As a community nurse we will develop your wound care, palliative care, and long‑term conditions knowledge and skills; supporting you to become an expert in assessment and care‑planning to deliver quality care that is caring and compassionate.

Full and part‑time hours available.

For this role you will be required to travel independently around the county, meeting strict time deadlines. You will need to hold a full UK driving licence and have use of a vehicle. Public transport is not acceptable for this role as it is not reliable and will not allow you to meet service needs. Please confirm in your application that you meet these criteria.

Qualifications
  • Have experience of working in the community
  • Are educated to RGN level 2
  • Can demonstrate a commitment to high‑quality care and service provision
  • Have the ability to work flexibly and as part of a team
  • Can assess and formulate care plans
  • Can teach and support student nurses and health care assistants
  • Have enhanced communication and interpersonal skills
Responsibilities
  • Keep the patient at the centre of care, ensuring participation in decision‑making for patient and carer
  • Provide a high standard of care and nursing intervention within a patient’s own home, with access to specialist and advanced practitioners when required
  • Work as part of a 24/7 service and multidisciplinary team
