Job Summary

Band 5 - Community Nurse. We are seeking a motivated and dynamic full-time Band 5 Community Nurse to work within the Integrated Care team in Brentwood and Billericay. The Brentwood and Billericay ICT is a forward-thinking and innovative service that is community based. The team works in a fully integrated way, with physiotherapists, occupational therapists, nurses, community matrons and care coordinators all working side by side to ensure an integrated and holistic approach to the care of people accessing our service.

Main Duties of the Job
  • Registered Nurse with experience of working in the NHS; community experience desirable but not essential.
  • Responsible for patients allocated to you and will assess, manage, plan and deliver care.
  • Support peers and senior nursing staff, be an effective team member whilst working on your own initiative.
  • Supervise pre-registration students and band 4 nursing assistants/assistant practitioners.
  • Commitment to collaborative working and actively supporting and liaising with other health and social professionals and agencies.
  • Computer literate as all patient records are now electronic.

About Us

Starting with NELFT. NELFT places a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary Period: This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High Cost Area Supplement – Fringe: This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (minimum payment of £1,258 and a maximum payment of £2,122 per annum pro rata).
