Job Summary

We are seeking a motivated, dynamic, and passionate Band 5 Community Nurse to work within our Mid Bedfordshire Primary Home Care Team, delivering responsive high‑quality nursing to patients living in Shefford and surrounding villages within Mid Beds. We are passionate about patient‑centred care, supporting our nurses to lead and deliver excellent and compassionate nursing and value our clinical teams as the real heart of the NHS. You will work closely with other members of the MDT to deliver care to patients with complex needs.

This is an excellent opportunity to develop your knowledge and skills within the community nursing setting. You will have the opportunity to be involved in facilitating self‑care for patients with long‑term conditions, health promotion and innovative activities and projects that enhance patient service delivery and staff well‑being. You will also have the opportunity to be involved in Urgent Response and Virtual Wards, which is a focus of our teams.

Responsibilities
  • Have a driving licence and access to a vehicle to carry out duties.
  • Work shift pattern covering 7 days a week, 08:00‑21:00.
  • Part‑time 30 hours per week fixed‑term post covering maternity leave.

Person Specification

Education

  • Registered General Nurse.
  • Demonstrated evidence‑based practice and up‑to‑date clinical skills.

Desirable Education

  • Evidence of post‑basic professional development.
  • Post‑registration qualification in a relevant clinical field.
  • Relevant teaching/mentorship qualification or commitment to undertake.
  • Post‑registration nursing experience, including community experience.

Physical/Mental/Emotional Support

  • Ability to mobilise, stand for most of the shift, kneel and crouch to deliver clinical care such as dressing wounds.
  • Ability to concentrate on drug calculations, tests, and manage interruptions and unpredictable patient/carer behaviour.
  • Occasional distressing or emotional circumstances such as imparting unwelcome news, care of terminally ill patients or safeguarding issues.
  • Work in occasional unpleasant conditions during home visits, encountering body odours, dust, noise/body fluid or verbal aggression.

Experience

  • Evidence of and commitment to multidisciplinary working.
  • Understanding of the interface between primary and secondary care.
  • Understanding of the roles of others in health and social care.

Desirable Experience

  • Familiarity with SystmOne for documentation in person‑centred care.
  • Proactive nurse with strong desire to lead on quality, holistic care delivery.

Knowledge

  • Literate in IT/computer skills.
  • Knowledge of clinical governance, safeguarding principles for vulnerable adults and children, health promotion and illness prevention.
  • Good oral communication skills, fluent in English.

Desirable Knowledge

  • Understanding of national government initiatives in health care.

Freedom to Act

  • Work within clearly defined clinical and professional policies.
  • Work independently, seeking appropriate support and advice when necessary.
  • Work within codes of practice and professional guidelines.

Other Essential

  • Current UK driving licence and access to a car.
  • Ability to travel across sites and Trust boundaries.
