Community Mental Health Nurse

Band 6 - £38,682 - £46,580 per annum

37.5 hours per week

Location: Taylor Centre, Warrior House, Southend

Contract: Fixed Term Contract/Secondment until 30 March 2026

The closing date is 28 December 2025

We are currently looking for an experienced, enthusiastic and dynamic Care Coordinator for a 5‑month period. As the successful post‑holder you will manage a caseload of service users with complex health and social care needs, following a recovery‑focused model. Previous care coordination experience is essential for this post.

Main duties of the job
  • Assessment, planning, implementation and evaluation of evidence‑based care, including health promotion for named service users/groups.
  • Admission, case conference/review and discharge in collaboration with the multi‑disciplinary team.
  • Coordinating care and assessment under the Care Programme Approach for a designated caseload.
  • Ensuring compliance with Safeguarding Vulnerable Adults and Children legislation.
  • Ensuring adherence to the Mental Health Act 1983.
  • Promoting carer and service user involvement within the service.
  • Establishing therapeutic relationships and implementing evidence‑based therapeutic interventions within boundaries set by the Nursing and Midwifery Council code of conduct.
  • Providing assessments, education and support to carers in line with NSF requirements.
  • Deputising and providing support and caseload coverage for colleagues/team members.
Additional responsibilities
  • Carry out mandatory training in line with Trust policy and procedures.
  • Participate in the staff appraisal process and manage staff if applicable.
  • Keep updated on Trust policy matters.
  • Provide management supervision when appropriate.
  • Work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi‑disciplinary team.
