In the role of the Community Clinical Nurse Practitioner you will be a key member of the unplanned Urgent Community Response Service providing an in‑depth assessment of nursing care and treatment for patients in their own home to prevent unnecessary admissions and/or facilitate timely discharge from hospital.
Hours of operation is 24 hours per day over 7 days per week within the UCR service. The successful candidate will join a team of UCR Clinical Practitioners with a wealth of clinical knowledge and experience who will support and encourage you in your new post. Successful candidate must have access to a vehicle for work purposes to meet the requirements of the role.
