Job description

We are looking for a motivated and compassionate Band 6 Community Care Coordinator to join the Croydon Assessment and Liaison Team at South London and Maudsley NHS Foundation Trust.

This is a varied and rewarding role where you’ll work with individuals presenting with complex bio‑psychosocial needs, completing high‑quality assessments, care planning, and short‑term interventions. You’ll be part of a supportive multidisciplinary team, working closely with partner agencies to ensure safe, effective, and person‑centred care. You’ll also play a role in supporting and supervising junior staff and students, and promoting service‑user involvement across all aspects of care delivery.

Main duties of the job

A & L is a fast‑paced but well‑supported environment, offering a different experience from a traditional CMHT. Caseloads are dynamic, with regular referrals and planned discharges, and service users are typically supported for up to 12 weeks. Clinicians hold a caseload alongside participating in a duty rota, which includes screening referrals, managing the duty phone, and completing initial screenings. The role offers excellent opportunities to develop strong assessment, formulation, and decision‑making skills within a collaborative team.

Job responsibilities

• Assess service users’ needs and plan, implement, and review care in collaboration with service users, carers, and the MDT
• Deliver person‑centred, holistic care that is inclusive and responsive to individuals cultural, social, and personal needs.
• Complete and maintain comprehensive risk assessments, ensuring they are fully documented, actioned, and regularly reviewed.
• Develop and review individualised care plans, providing ongoing assessment and follow‑up.
• Promote health, wellbeing, and recovery‑focused practice, supporting service users to actively engage in their care.
• Work in line with Trust policies, professional codes of conduct, and relevant legislation (e.g. MHA, CPA, safeguarding).

Qualifications

• What you’ll need:
• - Qualified healthcare professional with current professional registration in Social Work (SWE) or Occupational Therapy (HCPC).
• - Evidence of ongoing continuing professional development (CPD).
• - Substantial experience working in community mental health services (e.g. Home Treatment Teams, CMHTs) and/or acute inpatient settings.
• - Strong knowledge of evidence‑based interventions for individuals with complex mental health needs.
• Proven experience in completing comprehensive assessments, formulation, and care planning.
