A leading health services provider seeks a motivated Band 6 Community Care Coordinator for the Croydon Assessment and Liaison Team. We are looking for a motivated and compassionate Band 6 Community Care Coordinator to join the Croydon Assessment and Liaison Team at South London and Maudsley NHS Foundation Trust.
This is a varied and rewarding role where you’ll work with individuals presenting with complex bio‑psychosocial needs, completing high‑quality assessments, care planning, and short‑term interventions. You’ll be part of a supportive multidisciplinary team, working closely with partner agencies to ensure safe, effective, and person‑centred care. You’ll also play a role in supporting and supervising junior staff and students, and promoting service‑user involvement across all aspects of care delivery.
A & L is a fast‑paced but well‑supported environment, offering a different experience from a traditional CMHT. Caseloads are dynamic, with regular referrals and planned discharges, and service users are typically supported for up to 12 weeks. Clinicians hold a caseload alongside participating in a duty rota, which includes screening referrals, managing the duty phone, and completing initial screenings. The role offers excellent opportunities to develop strong assessment, formulation, and decision‑making skills within a collaborative team.
