A local government authority in Scotland seeks an individual to work with the Mental Health Officer team, focusing on assessments and supported decision-making for community members. The role requires engagement with individuals in both community and acute settings. Understanding social care support and knowledge of power of attorney options is essential. This temporary position offers a hybrid work model and competitive benefits.
This is an exciting opportunity to work alongside the statutory Mental Health Officer team as we enhance our supported decision making service. The post holder will undertake assessments for people who require or are likely to require a future social work service. The post holder will be required to work closely with other services to ensure we achieve early interventions and supports for people in the community and acute settings. The focus of the assessments undertaken will be to fully consider the persons decision making, how they can be supported with current and future decisions and undertaking anticipatory care planning.
You should have a good understanding of community social care support in order to support people to form anticipatory care plans. In addition you should have a strong working knowledge of options available to support people with their future decision making, primarily power of attorney's.
The post holder will promote supported decision making by attending local events and directly engaging with members of our community.
The post holder will also undertake assessments for people in hospital with the aim of supporting the person home at the earliest opportunity.
This role is focused on providing a person centred, early intervention and empowering service to ensure people are considering their current and future care needs.
