The Intensive Community Support for Dementia (ICSD East) offers an exciting opportunity for a full‑time Band 6 Registered Mental Health Nurse to join this dynamic team in a permanent position.

Team Overview

ICSD is a hospital‑avoidance team that aims to provide a high standard of patient‑centred care for people with a diagnosis of dementia to enable them to be cared for in their own home or other community settings. ICSD East is a specialist community dementia team, with medical and occupational therapy input, and is a great mix of autonomous and teamwork where everyone's contribution is valued. The team is based at Alderney Hospital and visits patients within the East of Dorset.

The team supports the Older People’s Community Mental Health Teams in managing their most complex cases and is highly effective in preventing unnecessary admissions and assisting safe planned discharges to and from the organic wards at Alderney Hospital. They do this through the provision of rapid, specialist and intensive short‑term, care‑planned support at home.

The team works closely with carers and other agencies in older people’s health and social care services and also supports the discharges of people with dementia from the organic wards at Alderney Community Hospital into community placements to ensure they are discharged effectively.

Key Responsibilities

Part of the Clinical Team Leader role is Gate‑keeping the organic beds at Alderney Hospital, Co‑ordinating the shift, ensuring all visits are covered, triaging new referrals and ensuring the patient is assessed within our timeframes. You will also be responsible for junior staff on shift and supporting them while they are on visits.

Who We’re Looking For

We are looking for an experienced registered nurse who wants a fulfilling role where they can really use their skills, knowledge and expertise to make a positive difference to people living with dementia in Dorset. Ideally we are looking for RNMHs but if you are an experienced RGN or RNLD with evidence of community and dementia experience then your application would be welcomed. You will need to be a dynamic and highly motivated nurse, with exceptional assessment, risk management and care formulation skills who has experience of working with older adults and dementia health in a multi‑disciplinary setting. You will have a very high standard of communication skills and clinical decision‑making, as this is essential in this role. ICSD is a ‘virtual ward’ and senior nursing staff must be able to process information in a variety of ways accurately and quickly.
