A regional healthcare provider in Walsall is seeking a Clinical Sister / Charge Nurse / Clinical Practitioner. Ideal candidates should be registered practitioners with experience in clinical settings and multidisciplinary teamwork. Responsibilities include assessing care needs, triaging service users, and implementing community healthcare programs. Opportunities have arisen for a Clinical Sister / Charge Nurse / Clinical Practitioner to join our innovative multi-disciplinary team. You must be a registered practitioner (nurse / paramedic) with evidence of post registration practice at Band 5. Experience of multi-disciplinary / multi agency working is essential. You will be part of a team and have responsibility for an urgent caseload triage and onboarding onto a virtual Ward as step up or step down to Community Services, working initially under the direction of senior nurses within care navigation centre. You will be working with Adult Services to develop and support this client group to remain in their homes with enhanced services to expediate hospital admissions and to avoid avoidable hospital admissions/ readmissions. You will need to work flexibly to meet the demands of the service and be able to forge strong links with partnership agencies at Social Services and Acute Trust.
Service provision is delivered over a 24/7 period across 7 days on a pro-rota system.

The £170 million development of our Pleck Road site was completed in 2010 and the continued upgrading of existing areas ensures the Trust now has a state-of-the-art Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit facilities. Work is also well underway on its multi-million pound new Urgent and Emergency Care Centre which will house a new Emergency Department (including Children’s Emergency Department), co-located Paediatric Assessment Unit, Acute Medical Unit and Urgent Care Centre. This is scheduled for completion this year.
We provide high quality, friendly and effective community health services from some 60 sites including Health Centres and GP surgeries. Covering Walsall and beyond, our multidisciplinary services include rapid response in the community and home-based care, so that those with long-term conditions and the frail and elderly, can remain in their own homes to be cared for wherever appropriate.
