A leading healthcare provider in the United Kingdom is seeking a Clinical Prescribing Pharmacist to manage chronic diseases and optimize medication therapy. This role includes conducting clinical medication reviews, offering medicines advice, and supporting Pharmacy Technicians. The ideal candidate will be registered with the General Pharmaceutical Council and have experience in primary care. This position provides opportunities for professional development within a supportive multidisciplinary team.
The postholder will be a Clinical Prescribing Pharmacist working within theirprofessional boundaries as part of a small, supportive, and multi-disciplinaryteam. This role involves both patient-facing and non-patient-facing functions.The post holder will be responsible for managing various areas of chronicdisease, including heart failure reviews, structured medication reviews (SMRs),hypertension management, lipid management, and other conditions based on theneeds of the practice.
The role alsoincludes conducting clinical medication reviews to proactively manage patientswith complex polypharmacy, ensuring optimal therapeutic outcomes. In addition,the post holder will help support the Pharmacy Technicians with clinicalqueries and will be required to make prescribing decisions for them whennecessary.
As part ofthe team's focus on safer prescribing practices, the post holder will offerexpert clinical medicines advice while addressing both public health and socialcare needs of patients.
The postholder will also be required to complete the Primary Care Pharmacy EducationPathway (PCPEP), if not already completed.

See patients with single or multiple medicalproblems where medicine optimisation is required
Review the ongoing need for each medicine including SMRs
Following discharge from hospitals to ensurepatients receive the medicines they need post discharge
Reading and analysing clinic letters from specialists to implement treatment plans
Ensure patients have appropriatemonitoring tests in place when required
Support MOIS, LCS and AMS projects outlined annually by localICB
Working at a strategic level across Foundry to optimise QOF andpromote best practice and quality improvement
Identification of cohorts of patients at high risk ofharm from medicines
Contribute pharmaceutical advice for the development andimplementation of new services that have medicinal components
Analyse, interpret and present medicines data tohighlight issues and risks to support decision making
Undertake clinical audits of prescribing in areasdirected by the PCN, feedback the results and implement changes
Implement changes to medicines that result from MHRA alerts
Implementation of Local and National Guidelines andFormulary Recommendations
Monitor practice prescribing against the local health policiesand guidance and make recommendations to GPs
Work with the general practice team to ensure thepractice is compliant with CQC standards where medicines are involved
Foundry is an innovative single Practice PCNserving a population of circa 28,500 patients from 4 sites across the Lewes andRinger area of East Sussex. The Practicehas received national acclaim for its population health management approach andour aim is to continuously improve the experience of care and outcomes for ourlocal population.
Undertake clinics, diagnose and treatpatients (within scope). Also, deal with appropriate patient presentation. Thiswill includebeing responsible for managing various areas of chronic disease,including heart failure reviews, structured medication reviews (SMRs),hypertension management, lipid management, and other conditions based on theneeds of the practice.
Provide pharmaceutical input for chronicdisease management, ensuring to make every contact count public healtheducation interventions with patients. Follow up identified patient groups,targeting better patient adherence with treatment to improve outcomes.
Identify patients requiring a medicationreview and undertaking a medication review in order to optimise drug therapy,reduce polypharmacy and minimise unnecessary waste, communicating directly withpatients as necessary (where appropriate, within scope of practice).
Responsible for managing long-termconditions, through conducting structured Medicine Reviews and Chronic DiseaseManagement clinics (within scope)
Review and action incoming hospitalcorrespondence, particularly where medicine reconciliation or initiation isrequired and managing medicines from transfer of care. This will includeliaising with local pharmacies to improve patient outcomes.
Provide advice on queries from otherhealthcare professionals
Advising patients of the adverse side effectof medication and potential interaction with other medication/treatments.
Provide medicines and long term diseaseadvice and support to GP practice staff, practice registered patients, and thewider practice team (within scope)
To advise on risk management processesrelating to the prescribing and monitoring of specialist drugs e.g. controlleddrugs and those subject to shared care arrangements.
Provide advice and support to improve thedisease registers to ensure all suitable patients are identified and offeredsuitable treatment and monitoring for their condition (within scope)
Depending on the needs ofthe practice, help GPs by managing the repeat prescribing reauthorisationprocess by reviewing patient requests for repeat prescriptions, issuingprescriptions and reviewing medicines reaching review dates and flagging upthose needing a review.
Ensure patients haveappropriate monitoring tests in place when required.
Facilitate and participate inmulti-disciplinary meetings related to prescribing topics and to engage in thetraining of other healthcare professionals and practice staff on mattersrelating to medicines usage.
Undertake clinical audits of prescribing indetermined areas, feedback the results and implement changes in conjunctionwith the relevant practice team.
To help with QOF, contributing to theachieving of Quality Outcome Frameworks objectives of Foundry Healthcare.
Prepare evidence-based resources andinformation to support the Clinical Pharmacy Team and all other relevant healthprofessionals in the implementation of rational cost-effective prescribing.
Support MOIS, LCS and AMS projects outlinedannually by local ICB, where appropriate.
