A leading healthcare provider in Greater London seeks a Clinical Pharmacist to join their team. The successful candidate will manage chronic disease medications, conduct clinical reviews, and provide support to GPs. Applicants should be registered with the General Pharmaceutical Council and have relevant experience in primary care. This role offers a competitive salary of £52,000 to £60,000 per year, with flexibility to work part-time. Opportunity for professional development and mentorship is provided.
Portland Medical Centre, as part of Bourne Health are looking for a proactive and enthusiastic Clinical Pharmacist who is already an established Independent Prescriber, to join our Practice to work onsite closely with our nurses and GPs.
The appointed Pharmacist would be part of a wider pharmacy team working with our associate practices within a Hub model.
As the role will develop the successful candidate will take responsibility for areas of Chronic Disease Management and undertake Clinical Medication Reviews, proactively manage patients with complex poly-pharmacy needs. The role would also entail providing primary support to GP staff with regards to prescriptions (this includes repeat prescribing), medication queries, complex clinical queries – please see full job description. We will look to provide support to develop and train the candidate to further their skills in their clinical interests. The Pharmacist would have an allocated GP Partner to mentor, support and guide them.
