A progressive medical practice in Slough seeks a qualified pharmacist to provide high-quality patient care. Responsibilities include conducting medication reviews, managing repeat prescriptions, and collaborating with healthcare teams. The ideal candidate will have excellent clinical knowledge, communication skills, and experience in a GP practice. The primary focus of the role is to work as anautonomous clinician, as part of Herschel Medical Centre, providing highquality and timely care to registered patients in Slough. Ensuring a highquality, professional clinical service to patients, doctors, staff, colleagues,health service professionals and others.
The role ofpharmacist will include but not limited to:
Participate and deliver education andtraining to practice staff of medication-related topics.
To present at patient group meetings orother appropriate events to give advice on the appropriate use of medicines.
To produce patient information leafletsand posters and run medicine awareness projects throughout the year.
To facilitate good working relationshipsbetween the practice and Community Pharmacies and Hospital Pharmacists.

Contributeto reductions in medicine-related hospital admissions and readmissions bysupporting patients to get the best outcomes from their medicines andidentifying and addressing medicines-related issues
Tosupport patients recently discharged from the hospital by ensuring theyunderstand their medication, any changes that have happened recently and havea timely supply.
Ensurethat the practice repeat prescribing policy is regularly reviewed and that itis being adhered to with the aim of reducing waste and improving safety.
Tohelp plan, develop and support the introduction of new working processes withinthe practice to optimise the quality and safety of prescribing.
Workingclosely with GPs to resolve day-to-day issues related to the medication.
Toact as a point of contact in the practice for all medication and prescription-relatedqueries from patients and health care professionals.
The role ofpharmacist will include but not limited to:
Structured MedicationReviews (SMR)
Tomanage the repeat prescribing reauthorisation process by reviewing requestsfor repeat prescriptions.
Participateand deliver education and training to practice staff of medication-relatedtopics.
Topresent at patient group meetings or other appropriate events to give adviceon the appropriate use of medicines.
Toproduce patient information leaflets and posters and run medicine awarenessprojects throughout the year.
Tofacilitate good working relationships between the practice and CommunityPharmacies and Hospital Pharmacists.
