A healthcare provider in Redbridge is seeking a qualified pharmacist to implement and manage a medicine management system. You will provide clinical assessment and management of patients on anticoagulation therapy, including prescribing and optimizing treatment according to guidelines. The ideal candidate has experience in anticoagulation management and strong communication skills, and is able to work both independently and as part of a multidisciplinary team. This position offers a unique opportunity to contribute to high-quality patient care.
Tobe responsible for implementing an effective medicine management system withinthe practice, including cost-effective prescribing, working as an independentprescriber, providing advice and support for both patients and colleagues. In addition,the post-holder will also support the multi-disciplinary team, in line with thestrategic objectives of the practice
Provideclinical assessment and management of patients on anticoagulation therapy(including initiation, monitoring, and dose adjustment).
Prescribeand optimize anticoagulation treatment in line with local and nationalguidelines.
Conductpatient consultations, review INR results, and ensure timely follow-up.
Maintainaccurate clinical records and contribute to audit and quality improvementinitiatives.
Collaboratewith GPs, nurses, and other healthcare professionals to ensure seamless patientcare.
Support education and counselling for patientsregarding anticoagulation therapy and adherence.

We are in the very fortunate position where all 4 of the Primary CareNetworks (PCNs) in Redbridge are working together as part of the Redbridge-wideGP federation Healthbridge Direct.
This means we are able to build strong clinical teams within andacross the PCNs. We are now at the very start of an exciting journey ofbuilding a team of over 100 additional clinical roles to support and enable thedelivery of services within each of our 4 PCNs. We are building teams withinand across the PCNs to enable peer support and team working both within andacross professions.
