Job summary

A healthcare provider in Redbridge is seeking a qualified pharmacist to implement and manage a medicine management system. You will provide clinical assessment and management of patients on anticoagulation therapy, including prescribing and optimizing treatment according to guidelines. Qualifications

  • Qualified Pharmacist with independent prescribing qualification.
  • Current registration with the General Pharmaceutical Council (GPhC).

  • Ability to work autonomously and as part of a multidisciplinary team.
  • Previous experience in primary care or community-based anticoagulation services.
  • Familiarity with clinical systems (e.g., EMIS, INR Star).

Tobe responsible for implementing an effective medicine management system withinthe practice, including cost-effective prescribing, working as an independentprescriber, providing advice and support for both patients and colleagues. In addition,the post-holder will also support the multi-disciplinary team, in line with thestrategic objectives of the practice

Main duties of the job

Provideclinical assessment and management of patients on anticoagulation therapy(including initiation, monitoring, and dose adjustment).

Prescribeand optimize anticoagulation treatment in line with local and nationalguidelines.

Conductpatient consultations, review INR results, and ensure timely follow-up.

Maintainaccurate clinical records and contribute to audit and quality improvementinitiatives.

Collaboratewith GPs, nurses, and other healthcare professionals to ensure seamless patientcare.

Support education and counselling for patientsregarding anticoagulation therapy and adherence.
