Clinical Pharmacist - Lewisham

Clinical Pharmacist (Part-Time or Full-Time)

A progressive and supportive primary care team in South East London is seeking an experienced Clinical Pharmacist to join its multidisciplinary team.

The practice serves a large and diverse patient population within a vibrant, multicultural community. Working in a modern, purpose-built health centre, the team is committed to delivering exceptional, patient-centred care while fostering an inclusive, collaborative, and forward-thinking working environment.

The Role

The Clinical Pharmacist will work as an integral member of the clinical team, which includes GPs, Advanced Nurse Practitioners, Practice Nurses, and Healthcare Assistants. The postholder will provide expert support in medicines optimisation, long-term condition management, and clinical leadership in safe and effective prescribing.

Key responsibilities include:

  • Conducting structured medication reviews for patients with long-term conditions and complex polypharmacy

  • Providing support and guidance to prescribers on safe, cost-effective prescribing practices

  • Managing repeat prescription processes and monitoring high-risk medications

  • Offering patient-facing clinics and telephone consultations for medication-related issues

  • Supporting the management of chronic disease areas such as hypertension, diabetes, and asthma

  • Providing education to patients to improve adherence and understanding of their treatment

  • Participating in audits, quality improvement projects, and clinical governance initiatives

  • Contributing to training and mentoring for colleagues and students

  • Working closely with the wider Primary Care Network (PCN) pharmacy team to ensure consistent and effective patient care

Person Specification

Essential:

  • GPhC-registered Pharmacist

  • Postgraduate qualification or experience in clinical pharmacy (e.g., in primary care or hospital setting)

  • Experience of conducting medication reviews and supporting long-term condition management

  • Strong communication, problem-solving, and clinical decision-making skills

  • Ability to work autonomously as well as part of a team

  • Commitment to patient safety, inclusion, and evidence-based practice

Desirable:

  • Independent Prescribing qualification or willingness to work towards it

  • Experience working within a GP practice or Primary Care Network

  • Knowledge of EMIS or similar clinical systems

  • Experience leading or contributing to QOF, prescribing incentive, or audit projects
