A local healthcare organization in England is looking for a motivated Clinical Pharmacist to join their team. In this role, you will be crucial in delivering patient care through structured medication reviews and collaborating closely with GPs and community pharmacies. The position offers a hybrid working model and a competitive salary ranging from £44,000 to £47,000 per year, based on experience. The ideal candidate should hold a Master's degree in Pharmacy and be registered with the General Pharmaceutical Council.
Knaresborough and Rural Primary Care Network (PCN) is looking for a motivated and aspirational Clinical Pharmacist to join us.
This is an exciting opportunity to be part of our well established and experienced pharmacy team working closely with GPs across the PCN and the wider locality.
The current PCN team already has excellent links with the local hospital, community pharmacies, care homes and other practice pharmacists in the area, so you would be truly part of a larger team working for better patient care.
The post holder would be based across two practices within the PCN and can be based fully in practice or a hybrid role with a minimum requirement to be in the practices.
The post holder will be critical in supporting the delivery of the Primary Care Network Contract Directed Enhanced Services (DES) specifications and will help lead improvements to maximise safe, cost effective best practice to improve the quality of patient care.
The role will provide expertise in clinical medicines management, provide face to face structured medication reviews, manage long term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, and acute prescription requests for patients.
Knaresborough and Rural PCN consists of six GP practices working together to provide safe, effective, high-quality care to improve the health and wellbeing of our local population. We share a commitment to joint working, whilst maintaining the continuity and identity that our communities value with each practice essentially acting as an inclusive multi-disciplinary team.
This role is employed in partnership with Yorkshire Health Network (YHN).YHN is the local federation for all 16 GP Practices across Harrogate and Rural areas, made up of fours PCNs. YHN and the PCNs work collaboratively to deliver a supportive structure which is focused on achieving the best results from a joint management and clinical delivery setup for the benefits of patients across the region.
