Job description
Clinical Fellow – Frailty

This post will support the service to deliver a high standard of medical clinical care as part of a team of Specialty Doctors and Consultants. The post holder will be responsible for undertaking patient assessments and delivering treatment for frail & older patients attending the emergency department then transferred to the Frailty Same Day Emergency Care and Frailty Decisions Unit. The objective is to help these patients return to their normal place of residency at the earliest opportunity.

Responsibilities
  • Participate in Consultant‑led ward rounds for the Frailty Decision Unit.
  • Identify patients in the Emergency Department suitable for Frailty SDEC.>
  • Complete a Comprehensive Geriatric Assessment under the direction of the Consultant.
  • Ensure medical care is provided in line with the agreed Medical Plan.
  • Discharge patients and provide timely paperwork to the General Practitioner to ensure a continuation of care following discharge.
  • Integrate discharge plans fully into wider community health and social services support plans.
  • Under direct clinical supervision from Specialty Doctors and Consultants, participate in Grand Rounds and ensure F1/F2 doctors receive a high standard of training.
  • Perform audits as outlined by the Educational Supervisor.
  • Comply with data protection and all relevant Trust Policies and Procedures.
  • Undertake additional duties in occasional emergencies and unforeseen circumstances at the request of the appropriate Consultant / Specialty Doctor. Duties arising under this sub‑section will not persist for prolonged periods or be on a regular basis.
Training and Development
  • Participate in Consultant‑led ward rounds and Grand Rounds.
  • Receive high‑standard training from Specialty Doctors and Consultants.
  • Audit participation as required by the Educational Supervisor.
