We would like to recruit an experienced Practice Nurse who is enthusiastic and highly motivated, with experience of chronic disease management in General Practice.
Main duties of the job
Key Responsibilities
- All treatment room duties including:
- Wound care and dressings
- Immunisations and vaccinations (including travel where required)
- Cervical cytology (smears)
- Chronic disease monitoring (e.g. diabetes, asthma, COPD)
- ECGs and phlebotomy
- Patient education and health promotion
- Accurate documentation and use of clinical systems (SystmOne)
Undertake the general duties of a Chronic Disease Nurse.
Be qualified and competent to be able to offer chronic disease management to support the practice in achieving quality indicators and targets of QOF, LES and DES including COPD/Asthma/Heart Failure/Diabetes.
To provide appropriate treatment to the practice population in accordance with practice protocols and procedures
Apply infection control measures within the practice according to local and national guidance.

Our Practice Team includes 3 GP Partners, 6 Salaried GP's, 1ACP, 1 Nurse, 1 Nursing Associate and 3 HCA's who work alongside 15 Clerical staff to provide excellent care for our 9300 patients in Swarcliffe, Leeds. We are part of the Seacroft PCN and are supported by their team of Pharmacists and social prescribers.
We are a training Practice and have a mix of Foundation Doctors and Registrars on rotation.
We look after 2 Care Homes and we believe in Continuity of Care for our vulnerable patients.
We are high QOF/LES/DES achievers and we always try to go the extra mile for our patients.

Job responsibilities
- a. Support the InfectionPrevention Control (IPC) lead and work to the IPC Policy
- b. Consult with patients and their families to undertake assessments ofpatient need
- c. Support in the long-term condition management within the organisation,including being involved with quality improvement and assurance initiatives
- d. Develop, implement and embed an effective care management programmefor the frail and housebound patients registered with the organisation
- e. Support public health and screening programmes, includingimmunisations, vaccinations, and cervical screening and also provideopportunistic health promotion advice
- f. Embed population health management approaches within general practice,targeting health inequalities and improving access
- g. As required, provideroutine nursing care to patients in accordance with clinical based evidence,NICE and the NSF
- h. Support new-to-practice nurses and other healthcare professionalswithin the organisation
- i. Teach and advise patients and their families on how to manage theircondition
- j. Participate in clinical audits and research projects and implementschanges as required, including supporting with the development, and updatingof practice protocols/guidelines and procedures locally
- k. Work within national and local protocols where these exist
- l. Recognise the boundaries of their practice and know when and to whompatients should be referred
- m. Support in the delivery of enhanced services and other servicerequirements
- n. Be aware of duties and responsibilities regarding current legislationand adhere to practice policies and procedures on Safeguarding Adults andSafeguarding Children
- o. Participate in the management of patient complaints when requested todo so and participate in the identification of any necessary learning broughtabout through incidents and near-miss events
- p. Undertake all mandatory training and induction programmes
- q. Contribute to and embrace the spectrum of clinical governance
- r. Support the practice team with the reviewing and implementation ofpractice policies and protocols, ensuring conformance to extant legislation
- s. Attend a formal appraisal with their manager at least every 12 months.Once a performance/training objective has been set, progress will be reviewedon a regular basis so that new objectives can be agreed
- t. Lead on public health campaigns (e.g., flu clinics)
- v. Provide wound care (ulcer/Doppler, etc.) to patients
- w. Maintain accurate clinical records in conjunction with extantlegislation ensuring that SNOMED CT codes are used effectively
- x. Contribute to practice targets (QOF, etc.), complying with relevantguidance
- y. Maintain chronic disease registers and implement an effectivecall/recall system
- z. Maintain a clean, tidy, effective working area at all times
- aa. Chaperone patients where necessary
- bb. Support the team in dealing with clinical emergencies
- cc. Liaise with external services/agencies to ensure the patient issupported appropriately (vulnerable patients, etc.)
There may be, on occasion, a requirement to carryout other tasks, this will be dependent upon factors such as workload andstaffing levels
Experience
- Experience of working autonomously
- Experience of infection prevention and control measures
- Experience of Practice Nurse skills including wound care, ECG, venepuncture, immunisations and new patient medicals.
- Good clinical system IT knowledge
- Understanding of safeguarding adults and children
- Broad knowledge of general practice and experience of working at a Practice
- Experience of Practice Nurse skills including womans health (cervical cytology, contraception etc.)
Qualifications
- Registered Nurse with the NMC
- At least 1 years experience working in a GP Practice environment.
- Qualified in Phlebotomy, ECG
- Qualified in a chronic disease management diploma (Asthma, COPD, Diabetes)
- Qualified in womans health (cervical cytology, contraception etc.)
