A healthcare trust in London is seeking a CHC Assessor to ensure individuals receiving funding from Continuing Health Care are assessed appropriately. Responsibilities include coordinating multidisciplinary assessments and reviewing care packages. The ideal candidate should possess a relevant nursing qualification and have knowledge of continuing healthcare assessments. The CHC Assessor will be responsible for ensuring that individuals who receive funding from Continuing Health Care are reviewed and assessed using nationally recognised tools. The post holder will review care packages and needs at prescribed intervals or when requested by care home managers where the client’s condition has changed. The CHC Nurse Assessor will coordinate multidisciplinary assessments for those eligible for a full assessment for fully funded NHS Continuing Healthcare by attending and presenting cases to the Complex Care Panel, and processing continuing healthcare referrals in accordance with national guidance. The post holder will carry out reviews and assessments for individuals who are self funding, jointly funded, or supported by social services.
To review clients after 3 months and thereafter yearly or sooner if necessary in accordance with the National Framework for Continuing Healthcare. To complete all relevant paperwork in a timely manner and inform the administrator at social services of the outcome of the assessments/reviews. To have current knowledge of the national criteria for fully funded NHS continuing healthcare. To work closely and collaboratively with the social services Residential Review Team to screen all residents in care homes using the nationally recognised checklist to ensure residents have the opportunity for a full continuing care assessment. To act as care coordinator for those residents who require a full continuing care assessment, coordinating the assessment and information from the multi-disciplinary team and presenting those cases to the Complex Needs Panel.
